Liposuction is a commonly requested cosmetic surgery. This is why liposuction, or liposculpture, is a popular procedure.
Dr. Roche is a member of the American Society of Liposuction Surgery Inc. and has been performing successful liposuction procedures for decades. He begins a tumescent liposuction procedure by inserting a cannula (a slim, hollow tube) into small incisions near the area to be re-contoured. The cannula is moved through the layers of tissue and extracts the necessary fat. Several techniques exist. The super-wet technique injects fluid into the patient’s body, roughly the same amount as the fat to be extracted. The tumescent liposuction technique injects double or even triple the amount of fluid. In UAL (Ultrasound-Assisted Lipoplasty), a specially designed cannula liquefies the fat and suctions it away. Except in the most extreme cases, procedures are performed on an outpatient basis. Dr. Roche will suggest the most appropriate form of anesthesia (epidural block, intravenous sedation or a local or general anesthesia).

Liposculpture (Ultrasonic-assisted)
Ultrasonic liposuction is a relatively new technique introduced into the United States for the removal of unwanted localized areas of fat using ultrasonic assisted liposculpturing. It offers many advantages over tumescent liposuction, including less post-operative bruising, swelling and discomfort as well as shorter recovery periods. In ultrasonic-assisted liposculpturing, an ultrasonic probe is inserted into the surgical site. Once this is accomplished, the fat is liquefied or rather emulsified by a physical process called “cavitation.” The cell membranes are disrupted allowing triglycerides and fatty acids to be released.
The former areas that were difficult to treat with liposuction, such as posterior hip and back, are particularly well addressed with ultrasonic assisted liposculpturing as is male gynecomastia and “enlargement of the male breast.” Other areas that are well treated with ultrasonic assisted liposculpturing are the central truncal areas, proximal extremities and the high hip areas. Dr. Roche has extensive experience in routine liposuction surgery and has had training in the use and the technique on the new ultrasonic equipment.

Echronia Laser-assisted Liposculpture
Dr. Roche is one of 10 surgeons in the country utilizing the Erchonia laser to improve the liposuction surgery results. He is involved in two national studies to further investigate the safety of liposuction procedures and also evaluate the new laser’s potential to make the liposuction procedure results even better with less pain and more rapid healing.
The laser is used pre-liposuction to soften the fat cells and possibly allow “lipo-shifting” or smoothing of lumps and depressions reduce pain, inflammation and bruising. There is no discomfort with the laser treatments. The post-operative laser treatments will accelerate healing and reduce pain and inflammation after any cosmetic surgery.
